I know he's been a prat, but I think it may be more likely that she's shot up and passed out and he's removing the needle. From the picture you don't even know if she is actually unconsious. If he is injecting her then he surely has to be arrested for assault.

Quite possibly a fake, how can we be certain that it isn't a bloke who looks like Doherty injecting a friend with insulin who has passed out due to diabetes, I believe very little that is printed in the Sun, other than the Horoscopes.
