Man U - Semis and Won league
Stoke - finalist and higest league finish in a generation
Man C - Winners and 3rd in premiership
BHA - Further in cup for generation and won league (easily)
Crawley - Went 6 rounds further than should have and won league (very very easily)

Nope, Fulham won the fair play Europa League spot.

This is how I understand the League cup to work (based on last year):

In the first round:
-All League 2 teams
-All League 1 teams
-All Championship teams, except two two highest league finishers last season (Birmingham and Blackpool)

In the second round:
-Winners of first round ties
-The two Championship teams handed byes in the first round (Birmingham and Blackpool)
-Premier League teams, except those playing in European competition (unless there are more than 6 Premier League teams in Europe)

In the third round:
-Winners of second round ties
-Remaining 6 Premier league teams
