Think I'm right in saying that although the Football League Review had a nominal cover price of one shilling, it used to come free as an insert (monthly?) In a club's normal matchday programme at no extra charge

That's correct. As a boy growing up in SE London I used to follow Charlton and always bought a programme (alongside all manner of programmes from the club shop - no Superstore then) Back in those days I could give you the names of at least half the squads of every League club such was my enthusiasm. Nowadays I can't even remember who won which competition for the last 10 years.
One of many reasons I hate the Premier League. Following football was so much better in the 60s and 70s for me
