It's always amazing to me how few people understand this concept.

Indeed,to try and make it easy for the less intelligent I always say what will the clock be showing in the first minute?

0= 1st Minute
1 = 2nd minute
2 = 3rd minutes

.
.
.
44 = 45th minute

So, being a bit pedantic, why was the applause started in the 45th minute and not the 46th minute (on the basis that Paul McCarthy was in his 46th year?

Not difficult to understand at all but there is a strong case for saying that, in these situations, it would make more sense for the applause to take place AFTER X number of minutes.. in which case, the clock would display the relevant number. Anyway, it worked okay so that's good.
