As someone who works in change for a large financial services company, you’d be amazed how often we stop people doing things that would bring the entire system crashing down. I wouldn’t rule out just useless developers being to blame.
I'd suggest that if the useless developers are allowed to make changes direct to live systems with no testing, the problem may lay elsewhere
