Southend were relegated for 2 suscessive seasons in 1997 and 1998. Wolves had suscessive relegations from the top flight to the old Division 4 in the 80s are the first ones that spring to mind.

Man City were relegated from Premier in 1996, they stayed in Division 1 for 2 seasons til 1998 when they were relegated to Division 2. So its not suscessive relegations.
