Not quite the same, but a small and seemingly innocouos thing, but something that keeps me awake in the middle of the night. Years ago I travelled to Dorchester for a work meeting. I met the person, we had a good conversation that lasted about an hour. We agreed to work together, I got up to leave and as we shook hands I looked her in the eyes and said 'hello'.
I'll never be able to explain it. My mind just got confused by the hand shake and thought we were meeting instead of departing.
It would have been OK if I'd owned it on the spot, laughed it off etc. But I just went 'erm' and she looked at me confused, and I turned around and walked off.
