I confirm that prior to attending each match of the 2021/22 season (select one):
I am double vaccinated and can produce a domestic NHS COVID Pass (or international equivalent) which will be valid at the time I attend and that the NHS COVID Pass (or international equivalent) will be readily accessible to show Club staff with corresponding photo ID upon request
OR
I will take a lateral flow test within 48 hours of each match, and log the results on the NHS website, only if I am negative will I attend and proof will then be readily accessible to show Club staff with corresponding photo ID upon request.

Declaration:

I understand that the law now requires me to have a valid NHS COVID Pass (or international equivalent) OR proof by NHS email/text message of a negative lateral flow test taken within 48 hours of the match, in order to attend a match, and if I am unable to show a valid NHS COVID Pass (or international equivalent) I will be refused entry to the American Express Community Stadium;
I will not attend a match if I test positive for COVID-19 in the 10 days prior to attending a match;
I will not attend a match if I have any recognised COVID-19 symptoms under the current NHS guidelines;
I will not attend a match if I am required to self-isolate under the current Government, NHS and Public Health England guidelines;
If any of the statements above are not true and accurate now or prior to the playing of a Match, I understand that I must not attend the American Express Community Stadium and that if I attend the match I will be refused entry and may face sanctions in accordance with the applicable ticket terms and conditions;
If I share my ticket using the season ticket sharing platform, I will only pass my ticket on to a guest who meets the above criteria and will ensure that they understand the above criteria are conditions of entry. I acknowledge that any guests who do not meet the above criteria may be refused entry.

To confirm the 'form' is electronic (not paper copy) and a link is in the email sent out. I'm not sure exactly what will happen if you have not completed the online form (I can see some fans missing this) presumably the fan number is updated on your match day ticket, season ticket app or physical plastic card and the ticket barrier knows you have or have not 'signed' the declaration? Having said all that its hard to believe all this can be done in such short notice ahead of Wednesday. I wonder if some fans will have to sign perhaps paper copies handed out by Stewards before gaining entry? Either way its a day to not leave entry to the last minute Im thinking!

Thanks for that. I’ve now done my declaration and it seems the friends and family who are using the tickets also received the email.

I missed the Email as it was buried others. I only picked up on it on Paul Barbers you tube video.Some people will miss it .

Whilst I think that the measures are a good idea they are not going to stop the spread of the new variant.

Just because your double / triple jabbed it doesn't mean you cant catch it / spread it.....

Be interesting to see if they start trying to enforce mask wearing on the concourse and lounges as it seems to have been pretty much abandoned at recent matches!

Don’t have a problem wearing it in these areas though I’m glad it’s not mandatory in your seat (though recommended). I’m not the only person who finds them very uncomfortable to wear for long periods, I’m sure…..

Edit: Have just found in the depths of my junk folder - had already given the once over following the above post but seem to get an inordinate amount of emails per day in there!

Had the email twice, the first one, sent yesterday evening, had an error in it apparently so they sent another at 12:15 today. So if you have already completed one, check again just in case you have another one to complete.
