wheels are tethered in F2, fact is even the strongest tethers dont always work thats a fact of life. If the wheel was'nt tethered in the first place it would come off a lot faster than it did, as a result it was just a freak accident and unfortunately thats part & parcel of car racing.

V8Supercars introduced a roll bar across the windscreen a few years ago, and it undoubtedly saved the life of Craig Lowndes at the Bathurst 1000 in 2005 from a flying wheel:

Surtees accident was sadly reminiscent of that at the 77 South African GP that killed Tom Pryce and a marshall.

Without permission, two marshalls decided to cross the track to aid a broken down car on the pit straight. The point they decided to cross was after a flat out blind kink. One made it but the other was struck at 175mph by Pryce's car. The marshall was killed instantly, 'ripped limb from limb' by the impact. If that wasn't horrific enough, the fire extinguisher that he had been carrying struck Pryce on the head with such force that the chin strap of his helmet partially decapitated him.

How about a small roll-bar (like you sometimes see on small convertibles by Porsche, Mazda or Toyota) that bends forward over the driver's head, and ends just an inch or so in front of the visor? A small piece of extra weight, but would work for larger objects such as wheels or fairings.
It would have to be easily removable for the driver to get out in event of a fire, though.

Sounds good but how is the driver going to get out of a car in a hurry if it's on fire? A roll bar above their head is likely to trap them, especially if they are unconcious and the marshalls are trying to get them out.

You cannot cater for every eventuallity. It is by its nature a very dangerous sport.
