personally think you're reading too much into it, max tops himself because he knows he will be topped before he gets to court , he hasnt been a gangster for the last 35 years anyway, hes been a corrupt politician/businessman.Yes he does know it's Max. It's when Noddles walks outside and the garbage truck comes along that it becomes a bit make your own mind up.
Is it a simile about Noddles avoiding the last 35yrs of garbage that Max has had as a gangster, or has Noodles had Max dealt with, or can't he be bothered.
I suppose the ending you imagine has a bearing on your own personality.

Have you noticed that the music 'Il Trio' - the three-way draw scene at the end of The Good, The Bad & The Ugly is being used more and more in movies and on TV. It's a kind of music box tinkling, before building to a crescendo - adding to the tension - and some awesome trumpet playing, all interspersed with looking at all three protagonists' pained expressions.
