Many of those needing treatment will have been of the "passed out drunk" variety, or trips and falls in the crowd. Out of 40,000 people, it's hardly out of the ordinary.
A total of 23 arrests were made, most for drunken behaviour, possession of class C drugs and a couple of minor assaults.
so your whole opinion is based on guesswork about how busy the town is on the night and the assumption that they could fit thousands more people into the narrow raods on the procession routes - shows just how ill informed you are.
The map below shows the main area that the spectators will line the streets, it's a relatively small area with limited capacity which is already stretched, so do you honestlyy think it would be a good idea to advertise it nationally and encourage more visitors on the night?
Would you rather have everyone able to attend and people getting badly hurt in the crush because at least then it wouldn't be seen as a local event or should all of the local population be thrown out of town so that people from miles away can come to watch

And yet, at an event with four times the number of arrests, the police say they're pleased the figure is so low. There are some double standards there.

If you weren't so blinkered in your view and opened your mind to rational thought once in a while you would be able to see why every effort is made to limit the size of the crowd at this event.
The societies don't want large crowds. The smaller the crowds, the more fireworks they can let off in the street and the more enjoyable the event is for those taking part - If only 1 man and his (deaf) dog was watching the event would still continue because it is not done just to simply pull in a crowd and entertain visitors.
There are still people that travel from across the globe to spectate, so i don't quite see why you think its locals only - it just sounds like you are jealous
It is an event that has lasted hundreds of years and will continue to be around for a very long time to come, no matter how many sad individuals like you moan and complain about it and the people taking part

Although I am no stranger to Lewes I have never been,or inclined, to visit, a Lewes Bonfire "celebration".
Lewes people are "special" and have never embraced the opportunity to share the "experience" and as such I am happy to let them engage in anti Papery without my company.

It is an improvement on the general behaviour of the locals,A town so bad that it has to have it's own 800 capacity prison. Brighton manages with 20 Cells in the Edward street Nick.

How many arrests are there on a typical friday night in somewhere where there are 40k revellers? somewhere like the clubbing areas in Brighton?
