Why they don't release extra tickets to all fans in line with the clubs release points? Its long past the date when the tickets should have been available to Bronze members

Because the initial allocation never made it down to that tier obviously.

How do these ' extra ' tickets suddenly appear. We got the initial allocation of 3000. It sold out. End of. West Ham haven't just released another 12 tickets! Can only assume we held a handful back for whatever reason and are now selling them.

The received wisdom (plausible, imo) is that they are returns from players, sponsors, and other club allocations.

A lot of the time its players friends and family. For Watford I picked up a last minute ticket and was stood next to Dick Knight. Which was nice.

Presumably there will always be a batch of tickets held back by the club for the use of guests of the Chairman, board etc, as well as of the players (including families etc). As the date of the match approaches it will become clearer exactly how many of those tickets will be taken up, and if there are spares they will then be released to the likes of you and me. It is, of course, a moot point, particularly in this current febrile atmosphere where everyone's getting excited about 'harvesting', the meaning of 'loyalty' etc whether the people for whom these 'extra' tickets are usually held are any more 'deserving' than us loyal, paying supporters who buy season tickets, attend away matches etc.

Agreed. It's the biggest remaining failing in the loyalty scheme. They used to do it in line with the original points allocation but stopped that a few games ago.
